On the Morning of Your Visit to CBST:
COVID-19 Screening Form
REGISTRATION:
Weekly Friday night services are OPEN TO ALL. Because of COVID-related safety protocols, seating capacity is limited. Therefore, in order to keep our community safe, CBST requires that each individual register in advance, and that each registrant be fully vaccinated.
The CDC considers a person is considered fully vaccinated 14 days after their second dose in a two-dose series, such as the Pfizer or Moderna vaccines, or 14 days after a single-dose, such as Johnson & Johnson’s Janssen vaccine.
If you do not meet these requirements, you are not fully vaccinated, and should not complete a registration.
Then, ON THE MORNING OF YOUR VISIT, you must complete a COVID Screening Form and submit it with your proof of vaccination, by 11am. (A smartphone pic of your vaccination card is perfect.)

ONCE INSIDE OUR BUILDING:
- Please arrive 15 minutes before services begin.
- Everyone must wear a mask at all times. Please note: Service Leaders will not be masked during services.
- Keep a moderate three-feet, social-distance apart at all times.
- When you arrive, check in with security at the front door.
- You will be guided to your seat in the Wine Family Sanctuary.
- Because you are wearing a mask, you will be allowed to sing.
- Restrooms are available on each floor.
- Once services have ended, find your way directly to the exit on the main level. Please do not congregate in the lobby.
